Generally, banks have their own standard format for L/C application.
So my best advice for you is to contact your bank for such request.

few days back i designed a new application form for my bank. i studied many form them. just google and you will find a lot. the form itself is not much interesting but it really matters how you set the terms and condition for the applicant.

Yes, It is very hard for me to set the terms and condition. As you do I collate the data and consult with our legal Dept. But now I have not yet finished.
